Record Book Forms
We have two formats available: PDFs or Microsoft Publisher.
PDFs require Adobe Acrobat Reader to open (If you don't have
Acrobat Reader you can download it for free at www.adobe.com.) After
printing them off you can hand-write on them.
With the Publisher version you can open the form in Microsoft
Publisher, enter information, and then print it off. However, be
careful to download the files for the right version of Publisher. The
Publisher 2.1 files (Pub 2.1) will work on almost any version of Publisher
before 2000. If you use Publisher 2000 or after, download the
Publisher 2000 files (Pub 2000).
Confused yet? Okay, summary:
If you don't have Publisher, download PDF files and open them with Adobe
Acrobat.
If you have a version of Publisher that is older than 2000, download the
Publisher 2.1 files. If you have Publisher 2000 or a newer version of Publisher, download the
Publisher 2000 files.

If you are one of the people in charge of updating the forms and want the
new forms on this page, please contact the webmaster!
*Tip: When downloading the Publisher or Word files, choose "save to disk"
and then open the saved file. Otherwise you will not be able to save your
changes to the file since it is directly on the website. You may have to
change the line spacing in some Publisher files, to do so select Format,
then select Line Spacing, adjust as needed. If you have any problem
downloading, viewing, saving, or opening these files please contact the
webmaster.

forms on this site. The forms on the state site are different and make it
harder to judge the books.

Emerald Star forms are only in Word and PDF.

Record Book forms will NOT be converted to Word anytime soon, if ever. So
don't be waiting for them to do your record book!

Also: The Ruby/Sapphire Star forms are in a JPEG (picture) format.
Meaning, you don't need Publisher, Word, or Adobe Reader. Also meaning
that you must print them out and write on them.
